Memberships forms are also available at the HHSIC.
Submit completed registrations form in person at the HHSIC.
A valid identification card and proof of current home/study
address will be required to validate the information.
Once membership form has been accepted, members will be issued
with a membership card that they must bring with them to be
able to use the HHSIC.
Membership starts only after the issuing of a membership
card.
Membership is valid for 12 months.
Any change of personal data should be reported to the HHSIC
as soon as possible.
Any loss of user card should be reported to the HHSIC immediately.
A replacement fee of 50 ZAR will be charged.
